As a veteran who has given some years of your life looking after the safety and security of your country, you want to know that your health needs will be looked after properly.  

We recognise the contribution you have made by offering bulk-billing to all DVA Gold Card holders. That way, you have the security of knowing that you will get the very best general practice care without incurring out-of-pocket costs.  

To make sure that our Veterans are being well looked after, we also participate in the Coordinated Veterans Care (CVC) Program.  

Coordinated Veterans' Care Program

The program is for veterans, war widows, war widowers and dependants who are Gold Card holders.   CVC Program criteria include the following

  • living in the community (not in a Residential Aged Care Facility)

  • have been diagnosed with one or more chronic conditions

  • have complex care needs, and/or

  • are at risk of unplanned hospitalisation 

A GP from our Practice or our Practice Nurse will help you assess your eligibility

 

Your personalised Care Plan

If you are eligible, your GP and nurse coordinator will develop your personalised Care Plan

  • detailed information about your medical history

  • goals and strategies for managing your condition

  • details of your medications

  • symptoms you should watch out for

  • appointments to specialists and other allied health providers 

You (or your carer) will be given a copy of your Care Plan. 

Your GP and nurse coordinator will work with you to regularly review your Care Plan, monitor your progress and, where necessary, take appropriate action.
